Module 1:Incident, Accident and Outline of Investigation Procedure

  • What are hazards and risk

  • Difference between “Incident” and “Accident”

  • Types of incidents

  • Legal aspects of incident/accident investigation

  • Gathering information

  • Analyzing information

  • Deciding immediate and root causes

  • Identifying suitable control measures

  • Planning corrective actions

  • Practical Exercise: Review a sample incident report and identify hazards, immediate causes, root causes, and propose corrective actions.

  • Case Study: Analyze a real-world accident (e.g., warehouse slip/fall) and discuss what went wrong and lessons learned.


Module 2:Interviewing Witnesses and Gathering Information

  • Preparation for the interview of witnesses and other concerned persons

  • Principles of effective interviewing

  • Separation of witnesses

  • Confidentiality of information obtained

  • Collecting factual information – photographs, sketches, videos, system alarms if any

  • Collection of required drawings, company policies, risk assessments, training records, SOPs, Safe Systems of Work, Permit-to-Work, Maintenance Records – need and importance of all this information

  • Practical Exercise: Conduct a mock witness interview based on a sample incident scenario; document the information collected.

  • Case Study: Evaluate witness interviews from a past incident report and identify gaps or errors.


Module 3:Analyzing Information, Identify Control Measures

Module 4 :Recording and Reporting of Incidents / Accidents

  • Practical Exercise: Draft an incident report using a provided scenario, applying legal requirements and best practices.

Module 5:Mock Accident Investigation

  • Mock accident case will be given to the class

  • Write down whom you will interview

  • Write down the interview questions

  • Write down what data you will collect

  • Your conclusion about immediate causes and root causes

  • Write down your conclusion about selection of control measures

  • Utilize the knowledge and skills gained during training and write an Accident Investigation Report

  • Discussion and Q & A session

  • Practical Exercise: Conduct a full mock investigation including interviews, data analysis, control measures selection, and report writing.

  • Case Study: Discuss a high-profile accident investigation (e.g., construction or industrial accident) and highlight key learning points.
