Module 2: Risk Assessment and Hazard Management

  • Hazard identification techniques: HAZID, HAZOP, and what-if analysis.

  • Job safety analysis (JSA) and task risk assessment (TRA).

  • Bowtie analysis for major accident hazards.

  • Case Study/Hands-on Exercise: Conduct a HAZOP study on a small process unit (e.g., a separator), identifying deviations, consequences, and safeguards.

Module 3: Process Safety and Operational Integrity

  • Process safety vs. personal safety.

  • Major accident hazards: fire, explosion, toxic release, and well control.

  • Barrier management and bowtie models.

  • Case Study/Hands-on Exercise: Develop a bowtie diagram for a well blowout scenario, identifying threats, barriers, and escalation factors.

Module 4: Permit-to-Work (PTW) and Isolation Management

  • Permit-to-work systems: types of permits and authorization.

  • Isolation, lockout/tagout (LOTO), and gas testing.

  • Coordination of simultaneous operations (SIMOPS).

  • Case Study/Hands-on Exercise: Review a permit-to-work for hot work on an offshore platform, identify missing isolation steps, and propose corrections.

Module 5: Emergency Response, Incident Investigation, and Environment

  • Emergency response plans: fire, oil spill, medical evacuation, and well control.

  • Incident investigation: root cause analysis (RCA), 5 Whys, and cause tree.

  • Environmental management: spill prevention, waste management, and emissions.

  • Case Study/Hands-on Exercise: Investigate a simulated incident (minor hydrocarbon spill), collect evidence, interview witnesses, and complete a root cause analysis report.
